On your first visit, you will be asked to fill out a confidential medical history form. From this form, your therapist will assess and discuss your treatment plan to meet your therapeutic needs. You can ask questions at any time throughout the treatment. At the end of treatment, your therapist may provide you with exercises for you to do at home.
